CLUB FORMS
In order to facilitate a well-organized, productive and beneficial club, below are forms either in Word, PDF, or Excel formats.
The club's Constitution (Word) , and the club's By-Laws (Word) , are required to hold a Charter and should be addressed at the beginning of each 4-H year for any updates or practical amendments. This also helps new members and leaders become familiar with your club and its policies. It also serves as a good reminder for those member and leaders re-enrolling.
The 4-H Unit Program Planning Guide (Word), or Program Planning Guide (PDF w/Fields) , (formerly Suggested Objectives and Goals) is designed to help each unit develop a plan of activities and goals that are appropriate to the needs of the community and unit membership. The unit officers should review this planning guide regularly with the membership to check on the club's progress.
In order to maintain your club's Charter and earn either a Gold, Blue or Red Seal, utilize the Monterey County 4-H Club Charter Award for (Word).
Enrollment in the 4-H Youth Development Program is open year-round; however, each club may have their own deadlines. The Monterey County 4-H Promotional Flyer (PDF Format) is a tool that can help increase your club's visibility with your community. It can be printed and copied to hand out at the your local schools and community events, during Nationa 4-H Week, etc.
The Officer's Manual (PDF Format), or Officer's Manual (PDF w/Fields) , is a guide for the 4-H club officers, the officer advisor, and the community club leader. Together this team is responsible for the administration of the 4-H club and its programs. Below are the individual forms that are in the manual.

MODIFICATION OF ENROLLMENT
To add or delete a project from your current enrollment, use the Monterey County 4-H Program Project Update Form
To enroll in a project that is being offered in a club other than your own, use the Monterey County 4-H Program Inter-Club Project Agreement
To request a transfer to enroll in a different club within Monterey County, use the Monterey County 4-H Program Request for Club Transfer
Please note that these changes are not valid until the forms are recorded at the 4-H Office. No changes to enrollment information will be made without all necessary signatures.
4-H INCIDENT REPORT
